- Position Type: Locum Tenens Nurse Practitioner, 1099 (PRN)
- Location: Douglas County, Oregon – Explore Oregon healthcare jobs in a region known for lush forests, rivers, and a welcoming small-town atmosphere.
- Schedule: 8-hour day shifts, 5 shifts per week
- Hours: 40 hours per week; minimum commitment of 30 hours per month
- Contract Duration: 26 weeks, with potential for extension
- Start Date: August 25, 2025
- End Date: February 21, 2026
- Estimated Weekly Salary: $4,110 - $4,870
Nurse Practitioner Qualifications
- Active, unencumbered Nurse Practitioner license in Oregon
- Minimum 1 year of clinical patient care experience required (3 years preferred, especially in primary care, adult/geriatric, or homecare settings)
- Previous experience in in-home risk assessment or home healthcare preferred
- Ability to travel within Douglas County (up to a 55-mile radius); reliable transportation required
- Strong proficiency with EMR systems
- Ability to work independently and communicate effectively with patients
- Bilingual skills are a plus
Key Responsibilities
- Conduct annual in-home wellness visits and risk adjustment health assessments for Medicare members with chronic conditions
- Perform comprehensive assessments, physical exams, medication reviews, cognitive and functional status evaluations, and social/environmental screenings
- Deliver patient health education on topics such as pain management and medication adherence
- Build trusting relationships with members through exceptional communication and empathy
- Assist in closing quality care gaps, including screenings and labs
- Document findings accurately and communicate effectively with the healthcare team
- Collaborate with coordinators and support staff to enhance clinical care delivery
